Rules, Customer Agreement and Release
**THIS FORM REQUIRES YOUR INPUT. PLEASE READ AND ACKNOWLEDGE ENTIRE FORM BY SCROLLING DOWN***
To create a safe and healthy environment for our guests, customers and staff members at Tail Gate must comply with and acknowledge the following:
Vaccinations:
All dogs must have current, up-to-date vaccinations. Owners must submit written verification from their veterinarian that their dogs have been vaccinated against DHLPP (distemper, hepatitis, leptospirosis, parainfluenza, parvovirus), rabies, bordetella and the H3N2 canine flu. We will send you email reminders when your dog’s vaccinations are due. It is your responsibility to provide on-going verification of current vaccinations. You may be charged a separate fee if it is necessary for our staff to secure verification.
Health:
All dogs must be in good health. Owners must certify that their dogs are in good health and have been free from any condition that could jeopardize other guests. Dogs that have had a communicable condition in the last 30 days will require veterinarian certification of health to be admitted or readmitted. During your dog’s stay, if it is detected that he or she has fleas or ticks, treatment will be applied and charged to you at a minimum rate of $30. At check-in all dogs are examined for signs of fleas and ticks. A dog will not be admitted if fleas or signs of fleas or ticks are noticed.
Collars
and name tags:
Quick-release collars and name tags are required.
Leashes:
Leashes must be either leather or nylon. Retractable leashes are not allowed.
Food:
All food brought to Tail Gate must be stored in an airtight rodent-proof container. Plastic baggies or opened dog-food bags are not acceptable. Food for overnight guests must be divided into portions. If it is not, our staff will portion the food, and a separate fee may be charged. This is to ensure proper amounts for each meal.
Behavior:
All dogs must pass Tail Gate’s behavior assessment before enrollment in our program. All dogs must be non-aggressive and non-food- or toy-protective. Owners must certify that their dog(s) have not harmed or shown any aggressive or threatening behavior toward people or dogs. Reminders: The safety and health of your pet is our main priority. Although it is supervised play, your dog still might acquire an occasional nip or scratch.
Age:
Any age dog is allowed at Tail Gate if he or she meets our criteria. Puppies may begin a daycare program before they receive their rabies vaccine and before they are spayed or neutered, if both will be completed by 6 months of age.
We reserve the right to admit unaltered dogs on a case by case basis and according to temperament.
Puppies younger than 4 months may not participate in group play, as they are going through a fear and imprinting stage. We do this to prevent any future behavioral issues. These puppies will stay in one of our suites and receive water, food (if desired), one-on-one attention from our staff and regular bathroom breaks.
Enrollment:
All guests must have a complete, up-to-date and approved enrollment form on file. ALL REQUIRED FORMS MUST BE SIGNED BY THE OWNER BEFORE
THEIR PET MAY PARTICIPATE IN ANY TAIL GATE PROGRAMS.
Fees:
Fees are due and payable at the end of each stay. Cash, check, MasterCard, Visa, American Express, Apple Pay, Google Pay and Android Pay are accepted. Payment for discounted packages must be in advance. There will be no refunds for unused portions of prepaid packages. We will always do our best to accommodate special needs of our guests; fees for special needs may be applied.
Hours of operation:
Mon - Fri: 6:30am–7pm
Sat: 7am–11am, and 4pm–7pm
